Revised Subaward Request Forms are now available on the Research Administration website (see the Forms page and/or the Subaward Agreements page). We encourage units to become familiar with these revised forms prior to August 1, 2022, as their use will be required at that time. Please replace all previously saved copies with this version (denoted with the revision date of “5/2022” in the footer) as after August 1st we will no longer be accepting prior versions.
How this form differs from the existing forms
- We have added a guidance page (see page 2) for each form to help address questions that come up. (This page may be removed prior to submission.)
- We have removed a few questions/requirements that are only needed in specific instances. The Contract Officer will reach out individually, as appropriate.
- We have made it more clear what documents are required, based on the type of payment method.
- We have added a section regarding Human Subjects Data - at times it may be possible to include data transfer terms within the subaward agreement rather than entering into an entirely separate agreement
- The certification has changed slightly – please review before adding your name.
