Research that requires special facilities, equipment, in-person contact with research subjects, access to protected data, or otherwise cannot be conducted remotely should be conducted on campus, provided that it may be conducted consistent with these guidelines.
Who needs to fill out this form: All faculty researchers and scholars, or their designees, who are planning to conduct research/creative activity on campus, as well as those planning to conduct fieldwork, community-engaged research projects, or human subject studies starting in summer 2020.
- The form should be filled out by one person per research/creative activity team or human subjects study if they take place in different locations. If a PI or faculty researcher oversees multiple labs or studies with human subjects, it should be filled out once for each location.
- If you need to access animal research facilities, please contact the director of the facility regarding access.
- Those who filled out and have been working under the “Essential Research” designation have until July 1, 2020 to fill out this form.
